Kitty Condos, LLC, Boarding Contract

1.  All cats must be current on their vaccinations.  Written proof must be provided for your file.
2. All cats must be parasite-free. If parasites are found, your cat will be treated immediately and
    charged to your account.
3.  Boarding fees are as follows:

4.  Cats will not be received on Sundays or Holidays.
5.  Check-out time is during business hours only.  Sunday releases are by appointment only.
6.  All charges are due and payable at the time you pick up your pet.  We accept Cash, Checks
     or Visa/MasterCard.
7.  Certain medications can be administered for a fee.
8.  If your cat has special needs, please tell us.

I certify that I am the owner of this cat.

I hereby grant permission to this boarding establishment to act in my behalf and in my pet's best interest, by obtaining veterinary care at my expense, if deemed necessary, for illness or injury.  I further agree to pay for all veterinary and other necessary services incurred by and for my pet during its stay in this facility.

This boarding facility agrees to exercise all due and reasonable care to prevent injury or illness to my pet.  However, in the event of illness or injury, the owners and employees of this boarding facility shall not be held personally liable for such injury or illness.

I agree to pay all costs for any property damage or personal injury caused by my cat during its stay.  I agree to pay all charges on the day of pick-up of my cat, and I understand that my cat may not leave the premises until all charges are paid in full.  I understand that any animal left for five days beyond the agreed date of pick-up may be sold or disposed of at the discretion of the kennel owner.

I have read and agree to the policy as stated above.
